PERSONAL INFORMATION WE COLLECT 

The information we collect may include: Your medical history, your personal information including your birth date, and contact details. These details are collected during the process of completing the initial intake forms and also during each consequent 1:1 consultation. 

This information is collected and is necessary to establish the state of your health, which provides me with the adequate information to best provide you with the holistic healing services offered.

We collect this information to best understand the most appropriate holistic healing services that most appropriately addresses your reason for treatment. This information gathering also advises Renee on any contraindications for any treatments of services provided.

INFORMATION COLLECTION FROM MINORS 

Sensitive information may be collected from children under the age of 18 under the following circumstances:

  • In the presence of their parents

  • with their parent or guardian’s full consent

When you visit the Site, we automatically collect certain information about your device, including information about your web browser, IP address, time zone, and some of the cookies that are installed on your device. 

Additionally, as you browse the Site, we collect information about the individual web pages or products that you view, what websites or search terms referred you to the Site, and information about how you interact with the Site. We refer to this automatically-collected information as “Device Information”.

We understand that some personal information is particularly sensitive. We will only collect sensitive information by methods that are reasonably secure, and necessary. The reason why we collect this sensitive information is to appropriately provide you with the services you have contracted for. 

Practitioners are subject to the following regulations regarding the collection of personal information, with a duty of confidentiality. You may choose not to provide us with your personal information. However, if you choose not to be completely honest with us, we may not be able to provide you with the services that you request. We are legally required to identify clients by collecting their name and address, and failure to provide this information means we cannot offer our services to you.

You may receive emails from Renee Gould or employees of Renee Gould including but not limited to our Administration Contractors and/or Australia Post and select courier companies via a Forms by Air which integrates the information to the online booking and software system Cliniko. HOW YOUR INFORMATION STORED

We are committed to securely storing and handling your sensitive information. All sensitive information is stored on a password protected computer, to Cliniko. We take reasonable physical, technical and administrative safeguards to protect your personal information from misuse, interference, loss, unauthorised access, modification and disclosure.

This information will be accessed by Renee for continual care each time you visit. This information is highly secured by two authentication settings and only accessed by Renee Gould - The treating practitioner.

ICLOUD STORAGE 

Some sensitive information may be stored securely online, or in the cloud through Cliniko. This information is only accessed by the treating practitioner, Renee Gould. You can find out more about their security provisions here: https://www.cliniko.com/policies/

SECURITY

We manage risks to your personal information by:

  • storing files securely

  • ensuring that only/key personnel have access to sensitive information

  • releasing information to service providers on a strictly need-to-know basis and conducting regular audits of our security systems.

DESTRUCTION POLICY: 

We keep all of your archived material for at least 7 years, there after that they are either fully secured in archived or destroyed.

From time to time we may combine information provided by you with information gathered from:

  • Instagram

  • Google Analytics

ACCESS TO INFORMATION

You can contact us to access, correct or update your personal information at any time. Unless we are subject to a confidentiality obligation or some other restriction on giving access to the information which permits us to refuse you access under the Privacy Act, and we believe there is a valid reason for doing so, we will endeavor to make your information available you within 30 days. When you place an order through the Site, we will maintain your Order Information for our records unless and until you ask us to delete this information.

We may update this privacy policy from time to time in order to reflect, for example, changes to our practices or for other operational, legal or regulatory reasons.

NOTIFICATION OF BREACH

If we have reason to suspect that a serious data breach has occurred and that this may result in harm or loss to you, we will immediately assess the situation and take appropriate remedial action. If we still believe that you are at risk, we will notify the Office of the Information Commissioner and either notify you directly, or if that is not possible, publicise a notification of the breach on this website.
